Hip pain or limited ROM

RF or ACPE

Other joint pain

At least 1 shoulder w/ subdeltoid bursitis and/or biceps tenosynovitis and/or glenohumeral synovitis and at least 1 hip w/ synovitis and/or trochanteric bursitis

Both shoulders w/ subdeltoid bursitis, biceps tensosynovitis or glenohumeral synovitis
